Trinity students living on campus are required to have a complete Health Record form on file. This includes a completed physical exam form as well as information regarding immunizations and health insurance coverage.
All full time students must submit the Health Record form to the Health Services before their initial registration at Trinity University. This Health Record includes the consent for treatment, signed by the student or parent/guardian, if the student is a minor.
We request your adherence to health record requirement, supported by the Texas Department of State Health Services, that students present proof of immunizations for diphtheria, tetanus, polio and measles, mumps and rubella.
Additionally, Meningococcal Vaccine is required by Texas state law and must be administered at least 10 days before the first day of classes.
All health records are confidential and will not be released without the specific written request of the patient.
Students requesting immunization records may submit the Request for Immunization Records to Health Services. Fill out the form completely and fax to Health Services at 210-999-8378 or mail to the address on the form.
Patients requiring other information from the health record may contact Health Services for information about how to obtain a copy.
